Document Checklist
Belgische identiteitskaart / Carte d'identité belge
Belgian identity card or residence permit
Where to get: Held by you after commune registration in the National Register
Required: Original — A, B, F, or H residence card per residence status
Communes verify that you are registered in the Rijksregister / Registre National at their address before accepting the application.
Rijbewijs / Permis de conduire / Führerschein
Original foreign driving licence
Where to get: Held by you; surrendered to the commune at issuance of the Belgian licence
Required: Original — photocopies are refused at the counter
If lost or stolen, a police certificate of loss or theft substitutes for the licence. The commune retains the surrendered foreign licence and, for non-EU/EEA exchanges, sends it back to the issuing country's authority once issuance is complete.
Beëdigde vertaling / Traduction jurée
Sworn translation of the foreign licence
Where to get: Sworn translator authorised by a Belgian court
Required: Only if the licence is not in Dutch, French, German, or English
Informal translations are refused. Allow €20–€50 per page of translation.
Pasfoto / Photo d'identité
Recent passport photo
Where to get: Authorised photo provider; white background, recent
Required: Required when your appearance differs from your ID document, or when the commune requests one
Foreign passport
Where to get: Held by you
Required: May be requested at counter to verify identity-document continuity (recognised non-EU/EEA route)
Geneeskundig attest / Certificat médical d'aptitude
Medical aptitude certificate (categories C and D)
Where to get: An authorised physician for heavy-goods or bus categories
Required: Only when exchanging to professional categories C, C1, D, or D1
Category B exchanges do not require this certificate at standard counter intake.
