Det Centrale Personregister / CPR-nummer
CPR-nummer (Danish personal identity number)
Issuing body: kommune Borger Service (or International House in Copenhagen, Aarhus, Odense, Aalborg)
Purpose: Required for folkeregister registration, MitID activation, and automatic yellow Sundhedskortet issuance
Allocated at the in-person CPR appointment together with default egen læge assignment under Sikringsgruppe 1.

Passport or national identity card
Required for: All applicants at the kommune Borger Service or International House appointment
Notes: EU/EEA/Swiss citizens may present a national identity card; non-EU citizens require a passport

Proof of address
Examples accepted: Rental contract, ownership deed, or a signed declaration from the registered resident at the address consenting to the applicant's registration
Required for: Folkeregister entry and the kommune's right-to-deliver the physical Sundhedskortet by post
The cardholder's name must match the name on the mailbox at the registered address. The postal carrier is required to return the card to sender on a name mismatch.

Issuing body: SIRI (Styrelsen for International Rekruttering og Integration)
Required for: EU/EEA and Swiss citizens establishing residence under EU Directive 2004/38/EC before CPR registration

Required for: Non-EU third-country nationals — work, student, family-reunification, asylum, or permanent residence pathway
MitID
MitID
Issuing body: Digitaliseringsstyrelsen via partner banks and ID-control providers
Purpose: Required for digital self-service on borger.dk — replacement-card requests, GP changes, Særligt sundhedskort applications, and EHIC orders
Activation is available immediately after CPR issuance, with an approximately 24-hour propagation window for the MitID app activation.
Særligt sundhedskort
Særligt sundhedskort (special health card for cross-border workers)
Issuing body: Udbetaling Danmark — International Sygesikring
Use: Documents the right to Danish healthcare for cross-border workers and posted workers not registered in the folkeregister, on equal terms with ordinary Danish residents while in Denmark
Det blå EU-sygesikringskort / EHIC
Blue EU-sygesikringskort (European Health Insurance Card)
Issuing body: Udbetaling Danmark for Danish-insured residents; the equivalent home-country authority issues the EHIC for inbound EEA visitors
Use: Documents the right to medically necessary state-provided healthcare in another EU/EEA country, Switzerland, or the UK during temporary stays
The blue card is not a substitute for the yellow Sundhedskortet domestically, and the yellow card does not work as an EHIC abroad.
