## Costs

- **Public insurance (employed, gross salary EUR 3,000/month):** EUR 260-280/month (Direct debit (split with employer)) — Employee pays ~8.8-9% of gross salary; employer matches. Total rate ~14.6% + Zusatzbeitrag (BMG 2026 average 2.9%; market average since 1 January 2026 is 3.13%, set independently by each Krankenkasse)
- **Public insurance (self-employed):** EUR 200-900/month (Direct debit) — Based on income; minimum ~EUR 200/month for low earners
- **Public insurance (students):** EUR 110-120/month (Direct debit) — Reduced student rate available until age 30 or 14th semester
- **Private insurance:** EUR 250-700/month (Direct debit) — Varies by age, health, and coverage level; only available if salary exceeds the Jahresarbeitsentgeltgrenze (EUR 77,400/year for 2026) or you are self-employed

## Steps

### 1. Determine Your Insurance Type

- Employed, salary at or below EUR 77,400/year (2026 Jahresarbeitsentgeltgrenze): must use public insurance (GKV)
- Employed, salary above EUR 77,400/year (2026 Jahresarbeitsentgeltgrenze): may choose public or private (PKV)
- Self-employed or freelancer: May choose public or private
- Student: Qualify for reduced-rate public insurance

> **If this fails:** Switching from private to public insurance later is very difficult, especially after age 55. If unsure, start with public — you can switch to private later but going back is hard.

### 2. Choose a Provider *(Krankenkasse)*

- Compare Krankenkassen by supplementary rate (Zusatzbeitrag) and services — all cover same base benefits
- TK (Techniker Krankenkasse) — English support, well-rated app
- AOK — Regional presence, many offices
- Barmer — Good digital services

> **Tip:** TK is the most popular choice among expats. English-speaking support, solid app, low supplementary rate. Visit tk.de/en to apply online.

### 3. Enroll with Your Chosen Provider

- Contact your chosen insurer online, by phone, or in person
- Online: Fill in the form, upload documents, sign digitally — approval in 1-3 days
- In-person: Visit a branch office with your documents

_Useful phrases:_
- "Ich möchte mich bei Ihrer Krankenkasse anmelden." (ikh merkh-teh mikh bye ee-rer kran-ken-kah-seh an-mel-den) — I would like to enroll with your health insurance. — use when Visiting a Krankenkasse office to sign up

### 4. Inform Your Employer

- Your insurer sends you a Mitgliedsbescheinigung (membership confirmation)
- Give this to your employer's HR department
- They need it for payroll deductions — they pay roughly half your premium

_Useful phrases:_
- "Hier ist meine Mitgliedsbescheinigung von der Krankenkasse." (heer ist my-neh mit-gleeds-beh-shy-nee-goong fon dehr kran-ken-kah-seh) — Here is my membership confirmation from the health insurance. — use when Giving your insurance confirmation to your employer

### 5. Receive Your Insurance Card *(elektronische Gesundheitskarte)*

- Your eGK card arrives by mail within 2-4 weeks
- Bring it to every doctor visit

> **If this fails:** Until your card arrives, ask your insurer for a temporary coverage certificate (Versicherungsnachweis) by email or app.

## FAQ

### What's the difference between public and private health insurance?

Public insurance (gesetzliche Krankenversicherung, GKV) covers you and your non-working family members for one premium based on salary. It is compulsory for employees earning at or below the Jahresarbeitsentgeltgrenze (EUR 77,400/year for 2026). Private insurance (private Krankenversicherung, PKV) offers faster appointments and more choice, but each family member pays separately and premiums increase with age. Most expats start with public insurance — it is simpler and more predictable.

### Can I choose any public insurance provider?

Yes. There are around 95 public insurers (Krankenkassen) in Germany and you can switch freely after a 12-month Bindungsfrist (§175 Abs. 4 SGB V). The base rate is set federally at 14.6% of gross salary; each Krankenkasse sets its own Zusatzbeitrag — BMG announced 2.9% as the 2026 average and the actual market average since 1 January 2026 is 3.13%. Popular choices for expats include TK (Techniker Krankenkasse), AOK, and Barmer — TK is widely cited for its English-language support.

### I'm a freelancer. Do I have to get health insurance?

Yes, health insurance is mandatory for everyone living in Germany. Freelancers can choose between public and private insurance. If you were publicly insured before going freelance, you can stay voluntarily. Public insurance for freelancers starts at about EUR 200/month for low earners and scales with income. Private insurance may be cheaper when you're young and healthy, but premiums increase with age.
