Required: Completed on the new Kasse's website, app, or paper form
Where to get: Directly from the chosen Krankenkasse

Per the post-2021 process, this single application is what triggers the switch. The new Kasse handles the notification to your previous one in the electronic Meldeverfahren.
Personalausweis oder Reisepass
Personalausweis or passport
Required: Original or copy depending on the Kasse — always required as identity proof
Cost: Already issued
Non-EU residents may also be asked for their Aufenthaltstitel; check the application form before submitting.
Steuer-ID and previous Krankenkasse details
Required: Eleven-digit Steuer-ID, plus name and member number of your current Krankenkasse
Where to get: Steuer-ID printed on payslips and tax assessments; current Kasse's name and number on your insurance card
Cost: Already issued
The new Kasse uses these details to process the switch electronically with the previous fund.
