Where to get: From your statutory or private Krankenkasse after enrolment
Required: Provide to employer for the joint social-insurance filing

Versicherungsnummernachweis
Existing Versicherungsnummer (if previously assigned)
Where to get: Earlier letter from Deutsche Rentenversicherung; replaces the legacy Sozialversicherungsausweis
Required: Only if you were insured in Germany before — the same number is reused
