Document Checklist
PPS number and dates of birth
Required: The Personal Public Service (PPS) number and date of birth of every person to be included on the card
Why it matters: The HSE cannot process the application without a PPS number for each named person, so allocate one first if you do not have it
Getting a PPS number is a separate, earlier step. If you have not yet been allocated one, complete that process before you apply for a card.
Income and savings evidence
Employees: A recent payslip dated within the last three months showing year-to-date figures
Pensioners: A State pension letter or a bank statement showing the pension, or a pension payslip from the last 12 months for an occupational or private pension
Self-employed: A Tax Return (Form 11) and Notice of Assessment
Savings and investments: Bank statements or certificates of interest, and share certificates where they apply
If your only household income is a social welfare payment, you qualify for a medical card and the income assessment is straightforward — state the payment frequency and amount on the form.
Expense evidence
Housing: A tenancy agreement, rent book or landlord letter for rent; a mortgage statement or provider letter for mortgage payments; a mortgage protection insurance letter
Childcare: A childcare provider letter stating the weekly cost and contact details
Travel to work: Your employment address and travel distance, a Vehicle Registration Certificate if you drive, or public transport tickets
Allowable expenses are deducted from your income before it is compared against the limit, so a household whose income looks high can still qualify once these are counted. Gathering them is the single most useful thing you can do to strengthen the application.
