The passport is the universal identity document accepted by Agenzia delle Entrate for cittadini stranieri. Bring both the original and one fotocopia.

Where to get: Italian consulate (visa); Ufficio Postale Sportello Amico (kit postale ricevuta for permesso di soggiorno in progress)
Required: Original or copy, depending on application status
If you are already inside Italy and have submitted a permesso di soggiorno kit at the post office, the ricevuta serves as proof of your residence-permit-in-progress for the codice fiscale request.

Required: Compilato (filled in) — request form for attribution of codice fiscale and tessera sanitaria
Modello AA4/8 is the canonical Agenzia delle Entrate form for codice fiscale and tessera sanitaria requests. It is referenced on the Agenzia delle Entrate scheda for duplicate requests and applies to first-time issuance as well.
