Required: Original — not returned after exchange
RDW: the gemeente surrenders your foreign licence and it is not given back.

Where to get: Authorised photo provider following the Rijksoverheid fotomatrix-2020 standard
Required: Original, recent, meeting fotomatrix-2020
Beëdigde vertaling
Sworn translation of the foreign licence
Where to get: Sworn translator in the Netherlands
Required: Only if your licence uses non-Latin characters (e.g. Japanese, Greek, Arabic, Cyrillic)

Gezondheidsverklaring
Gezondheidsverklaring (Health Declaration)
Where to get: Applied via Mijn CBR online or paper form; processed by CBR; once approved, a Verklaring van Geschiktheid is registered in the rijbewijsregister
Required: Required for non-EU/EFTA licence exchange; conditionally required for some EU/EFTA cases
RDW lists this as a required document for non-EU/EFTA exchange; CBR processing can take up to four months.
Beschikking 30%-regeling
30%-ruling decision (beschikking)
Where to get: Belastingdienst — held by you or your employer's HR
Required: Only if invoking the 30%-ruling exchange path
RDW: "Decision on Evidence Rule (if applicable under 30% tax rule)."
Oud Nederlands rijbewijs
Old Dutch driving licence
Where to get: Held by you, if applicable
Required: Only if you previously held a Dutch licence
