Documents: One original or certified document showing full legal name, date of birth, and lawful status β for example a certified U.S. birth certificate, a U.S. passport, an Employment Authorization Document, a Permanent Resident Card, or a foreign passport with an approved I-94
Group: Group A β one required
Must be an original or certified copy; mobile-phone photos and photocopies are not accepted
Proof of Social Security number
Documents: One document showing the full SSN β for example a Social Security card, a W-2, a recent pay stub or printed electronic deposit receipt showing name and full SSN, an SSA-1099, or a Non-SSA-1099
Group: Group C β one required for SSN-eligible applicants
An applicant ineligible for an SSN is not required to present this document and instead signs a declaration of ineligibility at the time of application
Proof of Illinois residency
Documents: Two documents β for example a bank statement, a rental agreement, a pay stub, a utility bill, or a voter registration card
Group: Group D β two required
Bringing only one residency document when two are required is a common reason an application is turned away
Proof of written signature
Documents: One document carrying your signature β for example a current non-REAL ID driver's license, a credit or debit card, a passport, or a canceled check
Group: Group B β one required
Signature comparison is part of the verification process
Proof of vehicle liability insurance
When needed: Required to take the road (driving) exam
Bring this to the facility on the day of the road test; the road test cannot proceed without it
