Document Checklist
Proof of identity totalling six points
Rule: You must present documents that total six points or more, and at least one of them must carry your signature. Each acceptable document is assigned a point value.
Where the exact points are published: The full list with each document's point value is in the DMV printable instructions (How to Apply, form ID-44) and the DMV's interactive Document Guide. Check those for the precise points your documents carry.
The proof-of-identity documents also establish your date of birth.
Social Security number
Requirement: Provide a Social Security number, or β if you are not eligible for one β proof of your ineligibility.
Proof of New York State residency
Standard license or permit: 1 proof of New York State residency.
REAL ID or Enhanced credential: 2 proofs of New York State residency.
Accepted examples: A recent bank statement or pay stub showing your current New York State address, among other documents.
A bank statement or utility bill must have been issued within the last 365 days, the address must be pre-printed (a P.O. box is not accepted), and any electronic statement must be printed.

Pre-licensing Course Certificate (MV-278)
When needed: Issued when you finish the pre-licensing course; the original is required at the road test.
Bring the original β copies are not accepted.
