Required: Completed in French, signed; submit to the local CPAM (régime général) or MSA (régime agricole)

Page 2 of the form lists the justificatifs to attach (liste des justificatifs à fournir selon la situation du demandeur). Workers do not file this form themselves: their employer triggers affiliation via the Déclaration préalable à l'embauche (DPAE).

Birth certificate (full / with parentage)
Required: Original. CLEISS lists languages it accepts directly (German, English, Danish, Croatian, Spanish, Finnish, Hungarian, Italian, etc.); other languages require a sworn-translator (traducteur assermenté) translation
Cost: Free for the underlying civil-status extract; sworn-translator fees vary
An incomplete or untranslated birth certificate is one of the most common refusal causes during PUMA enrolment.
