Limited Stay Permit Card or Permanent Stay Permit Card
Where to get: Issued by Direktorat Jenderal Imigrasi as a separate process before this one
Required: Original card in hand, plus a clear scan or photo for Mobile JKN identity verification
Cost: Free at the BPJS step (KITAS itself is paid separately at Imigrasi)
Perpres No. 82 Tahun 2018 Pasal 6 ties JKN participation to working at least 6 months in Indonesia. KITAS issuance is the practical proof of that residency status.

Required: Original (not expired); scan of bio-data page for Mobile JKN registration
Cost: Free at the BPJS step
Nomor Kartu Keluarga (KK)
Family Card number (if enrolling dependents)
Where to get: Dukcapil (Direktorat Jenderal Kependudukan dan Pencatatan Sipil)
Required: KK number entered into Mobile JKN at registration; physical card not uploaded
Mobile JKN's PBPU self-registration assumes a KK number. Foreign residents without a KK on hand may need to register through a BPJS Kesehatan Kantor Cabang branch office instead of the app.
Active mobile phone number
Where to get: Indonesian SIM, registered to your name
Required: Used for OTP verification during Mobile JKN registration
Active email address
Where to get: Self
Required: Used for participant agreement and Virtual Account delivery
Buku rekening
Indonesian bank account or passbook
Where to get: One of: BNI, BRI, BTN, Mandiri, BCA
Required: Account number entered into Mobile JKN if enabling auto-debit
Auto-debit is the BPJS Kesehatan-supported recurring payment method. Manual payment via the issued Virtual Account at any of these banks is also accepted.
