Enrolling in SGK General Health Insurance as a Foreign Resident in Türkiye
Foreign residents in Türkiye move through two health-insurance regimes.
During year 1 of ikamet izni (residence permit) the holder must keep a private health-insurance policy meeting Hazine ve Maliye Bakanlığı Circular 2014/9 (updated by Circular 2021/8). Once continuous legal residence exceeds one year, Article 60(d) of Law 5510 automatically makes the foreigner a compulsory Genel Sağlık Sigortası (GSS) insured of the Sosyal Güvenlik Kurumu (SGK), unless already covered as a salaried employee, self-employed, public officer, or under another country's social-security system. Bildirim to SGK is due within one month; the premium is 6% of the gross asgari ücret.
Estimated time
Same-day notification at the local SGK office once the one-year residence threshold is crossed; e-Devlet status check available within minutes for residents already automatically registered
Cost
Current GSS monthly premium under Article 60(d) is 6% of the gross asgari ücret — see costs and additional items for the current figure; year-1 private-insurance substitute varies by SEDDK-licensed insurer

Additional Items
- The 2026 monthly GSS premium under Article 60(d) is ₺1.981,80 — 6% of the gross asgari ücret of ₺33.030,00 in force from 1 January 2026. The 6% rate has applied since 1 December 2021, when Law 7256 amended the prior 3% rate.
- Each annual asgari ücret revision flows through to the GSS premium automatically — no separate SGK announcement is required. Mid-year minimum-wage revisions (when enacted) take effect on the announced force-date.
- Premium is monthly, due by the end of each month for that month's coverage. Unpaid premiums accumulate as prim borcu visible in e-Devlet; sustained non-payment suspends benefit access until the balance is settled with the statutory gecikme zammı (delay surcharge).
- Dependents under Article 3(10) of Law 5510 — eş (spouse), çocuk (children up to 18, 20 if in non-higher vocational education, 25 if in higher education, lifetime if certified disabled), and ana-baba (parents) — are covered without additional premium. Parent coverage is conditional on the parent's total income being below the net asgari ücret, no other child already supporting them via SGK, and no independent GSS or pension entitlement. Foreign dependents must each hold a YKN and a residence permit.
- Çalışma izni (work permit) holders are insured under Article 4(1)(a) (salaried) or 4(1)(b) (self-employed) from day one of employment, with the GSS premium bundled into the employer payroll deduction. The Article 60(d) voluntary GSS path applies only to year-2-and-beyond foreigners not in Article 4 categories — typically students, retirees, family-permit holders, and self-supporting long-stayers.
- Coverage at SGK-contracted private hospitals carries an ilave ücret (supplementary fee) of up to 200% of the SGK reference price, charged by the hospital to the patient on top of the SGK reimbursement. Public state hospitals and university hospitals charge no ilave ücret.
- Long-term residence permit holders (granted under Article 42 of Law 6458 after eight uninterrupted years of legal residence) automatically hold full Article 60(d) GSS rights and do not separately require private insurance.
- MHRS appointment booking is available in Turkish, Arabic, English, and Russian via Alo 182, the mhrs.gov.tr portal, the mobile app, or the e-Devlet integration. Aile hekimi (family physician), specialist, and dental appointments at Sağlık Bakanlığı hospitals route through MHRS.
Step-by-Step
- 1
Confirm continuous residence and category
- Check the ikamet izni belgesi (residence permit) to identify the issue and validity dates
- Print the Adrese Dayalı Nüfus Kayıt Sistemi (ADNKS) address record from e-Devlet to confirm the registered Turkish address
- Determine whether you are insured under Article 4 of Law 5510 (salaried, self-employed, or public officer) — if so, GSS is bundled with payroll and Article 60(d) does not apply
- Confirm you are not insured under your country of origin's social-security system; if a bilateral treaty applies, prepare the home-country Certificate of Coverage
💡 Tip: Article 60(d) status is ipso jure — it arises automatically on the day the cumulative-residence clock crosses one year. No application creates it; bildirim only records it.
⚠️ Watch out: If the residence permit shows a discontinuity (a previous permit lapsed before a new one was issued), the cumulative-residence clock resets. Verify the discontinuity at the Göç İdaresi district office before assuming Article 60(d) applies.

Hold a Circular 2014/9-compliant private policy during year 1
Özel sağlık sigortası
Expat New ArrivalYear-1 residents — short-term ikamet izni holders before crossing the one-year threshold
- Choose a SEDDK-licensed insurer offering policies that meet Hazine ve Maliye Bakanlığı Circular 2014/9 minimums (updated by Circular 2021/8)
- Verify the policy carries the standard-form declaration referencing Circular 2014/9 (or 2021/8) — Göç İdaresi checks for this verbatim text
- Maintain the policy for the full validity period of the ikamet izni being issued or renewed
- Renew the policy on each ikamet izni extension until the one-year residence threshold is crossed
💡 Tip: There is no closed accredited-insurers list. Any SEDDK-licensed insurer issuing a compliant policy qualifies — compare premiums and coverage among the active providers in the segment.
⚠️ Watch out: If the policy is silent on Circular 2014/9 compliance, Göç İdaresi may refuse to issue or renew the ikamet izni. Ask the insurer for an updated policy carrying the standard declaration before the residence-permit appointment.

Check tescil status on e-Devlet at the one-year mark
- Log in to turkiye.gov.tr using the YKN and e-Devlet şifresi
- Open Genel Sağlık Sigortası Tescil ve Prim Borcu Sorgulama (turkiye.gov.tr/sgk-gss-borc-dokumu)
- If the page shows tescil under Article 60(d) with a start date matching the threshold date, no in-person filing is required — proceed to premium payment
- Check the dependents query (sosyal-guvenlik-genel-saglik-sigortalisinin-bakmakla-yukumlu-oldugu-aile-bireyleri-sorgulama) to see which eş, çocuk, ana, or baba are linked
💡 Tip: 2026 SGK announcement: "2026 yılı için Genel Sağlık Sigortası aylık prim tutarı ₺1.981,80'dir (asgari ücretin %6'sı)." The figure shown on e-Devlet matches this rate from 1 January 2026 and updates automatically with each asgari ücret revision.
- 4
File the bildirim in person at SGK if not auto-registered
Genel Sağlık Sigortası Giriş Bildirgesi
If e-Devlet does not show tescil within a few weeks of the threshold date
- Identify the SGK Müdürlüğü responsible for the ADNKS-registered address
- Bring the ikamet izni belgesi, YKN, ADNKS address printout, passport, and a written self-declaration that you are not insured under another country's social-security system
- File the Genel Sağlık Sigortası Giriş Bildirgesi at the counter — same-day processing in most directorates
- The bildirim window is one month from the threshold date under Article 61 of Law 5510
⚠️ Watch out: If the SGK Müdürlüğü does not accept the bildirim (e.g. citing an unclear ADNKS record or missing self-declaration), correct the deficiency and resubmit within the one-month window. Premium liability runs from the threshold date regardless of filing date, but late filing exposes the resident to gecikme zammı (delay surcharge) on overdue premiums.
- 6
Register dependents and use SGK-network care
- Add eş, çocuk, and any qualifying ana-baba dependents to the SGK record — each foreign dependent needs a YKN and a residence permit first
- Verify each dependent in the e-Devlet dependents query
- Use MHRS (Merkezi Hekim Randevu Sistemi) to book appointments at Sağlık Bakanlığı hospitals, Ağız ve Diş Sağlığı Merkezleri, university hospitals, and aile sağlığı merkezleri
- Patient katılım payı (co-payment) at SGK-contracted facilities is 10% on active-insured prescriptions, fixed amounts on ER and specialist visits; emergency care is unconditional
💡 Tip: Coverage at SGK-contracted private hospitals carries up to a 200% ilave ücret (supplementary fee) on top of the SGK reference price. Public state hospitals and university hospitals charge no ilave ücret.
Local Tips from the Community
- The one-year clock is the foreigner's cumulative legal residence in Türkiye, anchored against the Adrese Dayalı Nüfus Kayıt Sistemi (ADNKS) record. Re-registering a foreign address in ADNKS suspends Article 60(d) status.
- Check e-Devlet first before going to the SGK office. Many year-2 residents are already auto-registered under Article 60(d) — the Genel Sağlık Sigortası Tescil ve Prim Borcu Sorgulama service shows the status and any premium debt without a counter visit.
- Article 60(d) GSS status is not optional. Holding a private policy does not exempt the foreigner from the premium once one year of residence is crossed — many year-2 residents drop the private policy and rely on the GSS network instead.
- Acil sağlık hizmetleri (emergency care) is provided to everyone in Türkiye irrespective of insurance status under Article 75 of Law 5510 and the Acil Sağlık Hizmetleri Yönetmeliği. Hospital staff settle the bill with SGK after the fact for GSS-insured patients.
- Merkezi Hekim Randevu Sistemi (MHRS) appointment booking requires only a Yabancı Kimlik Numarası (YKN) and a valid residence record — GSS-insured status is not required to book. Without GSS, the consultation is billed to the patient unless paid through another insurer.
What Could Go Wrong
Cross the one-year residence threshold: e-Devlet does not show automatic tescil under Article 60(d) within a few weeks
Recovery: File the Genel Sağlık Sigortası Giriş Bildirgesi in person at the SGK Müdürlüğü responsible for the ADNKS-registered address. Bring the residence permit, YKN, ADNKS address printout, and a written self-declaration that you are not insured under another country's social-security system. The notification window is one month from the threshold date under Article 61.
Pay the monthly GSS premium: Premium not paid by month-end — prim borcu accumulates and benefit access is suspended
Recovery: Settle the outstanding balance plus statutory gecikme zammı (delay surcharge) via the SGK şahıs online payment portal (gss.sgk.gov.tr/SahisOdemeOnlineBasvuru/), bank, PTT, or e-Devlet. Benefit access reinstates on full settlement. Set up direct debit at the bank or via e-Devlet to avoid recurrence.
Renew the year-1 ikamet izni: Private health-insurance policy does not carry the Circular 2014/9 standard-form declaration and Göç İdaresi refuses the renewal
Recovery: Request an updated policy from the SEDDK-licensed insurer carrying the standard declaration "İşbu poliçe ikamet izni başvurularında yaptırılacak özel sağlık sigortalarına ilişkin 06.06.2014 tarih ve 9 sayılı Genelge'de belirlenen asgari teminat yapısını kapsamaktadır" (or the equivalent referencing Circular 2021/8). Any SEDDK-licensed insurer offering compliant policies — the segment is open — can issue a replacement.

FAQ
Documents
I have a work permit and a salary — do I file Article 60(d)?
No. Salaried employees, company directors, self-employed persons, and public officers are insured under Article 4 of Law 5510 from day one of qualifying employment — the işveren prim (employer-side payroll contribution) bundles GSS, old-age, invalidity, survivors, and unemployment-insurance contributions in a single deduction. Eş, çocuk, and qualifying ana-baba are automatically covered as dependents. Article 60(d) applies only to foreigners not in any Article 4 category — typically students, retirees, family-permit holders, and self-supporting long-stayers.
Costs
What is the GSS premium in 2026?
₺1.981,80 per month — 6% of the gross asgari ücret of ₺33.030,00 in force from 1 January 2026. The 6% rate has applied since 1 December 2021, when Law 7256 amended the prior 3% rate. Each annual asgari ücret revision flows through to the premium automatically — when the minimum wage is revised mid-year (as it has been in some recent years), the GSS premium revises on the same force-date.
General
When does my GSS-insured status start?
Article 60(d) GSS-insured status arises automatically on the day the foreigner's cumulative legal residence in Türkiye crosses one continuous year, provided the person is not insured under Article 4 (salaried employee, self-employed, or public officer) and is not covered by their country of origin's social-security system. The status arises ipso jure on the threshold date — no application is needed to trigger it. Bildirim (notification) must be filed with SGK within one month of the threshold date under Article 61 of Law 5510. Premium liability begins on the threshold date regardless of when the notification is filed.
Do I still need private health insurance after one year of residence?
Article 60(d) GSS coverage is mandatory once the one-year residence threshold is crossed for foreigners not in Article 4 categories; the GSS premium is owed irrespective of whether a private policy is in force. Many year-2 residents drop the private policy at that point because the GSS network covers state hospitals, university hospitals, and SGK-contracted private hospitals. Some retain a private policy in parallel for access to non-SGK-contracted private hospitals with lower out-of-pocket cost; this is optional. Year-1 residents do still need private health insurance — under Articles 8 and 23 of Law 6458, every short-term ikamet izni requires a SEDDK-licensed policy meeting Circular 2014/9 minimums.
Are my spouse and children covered automatically?
Yes — Article 3(10) of Law 5510 extends coverage from the primary GSS-insured to eş (spouse) automatically while the marriage is registered, and to çocuk (children) up to age 18, up to 20 if in non-higher vocational education, up to 25 if in higher education, or lifetime if certified disabled. Foreign-national dependents must each hold a Yabancı Kimlik Numarası (YKN) and a residence permit before being added to the SGK record. For foreign-national children over 18, an educational-enrolment document is also required to extend coverage into the 18-25 window. Verify each dependent via the e-Devlet dependents-query service after enrolment.
What about my parents?
Ana-baba (parents) coverage under Article 3(10) is conditional. A parent qualifies only if the parent's tüm gelir toplamı (total income from all sources) is below the net asgari ücret, no other child of the parent is already supporting them through SGK, and the parent does not hold an independent GSS or pension entitlement. Each foreign-national parent must also hold a YKN and a residence permit before being added. Article 60(d) foreigners with a self-supporting residence permit generally meet none of the conditions on the parent's side, so parent dependence is uncommon in practice but available where the conditions are met.
I'm a tourist — am I covered by GSS?
No. GSS coverage under Article 60(d) requires both a Turkish residence permit and continuous legal residence exceeding one year — neither applies to a tourist. Acil sağlık hizmetleri (emergency care) is, however, provided to everyone in Türkiye irrespective of insurance status under Article 75 of Law 5510 and the Acil Sağlık Hizmetleri Yönetmeliği. Tourists facing non-emergency care pay the hospital tariff or rely on private travel insurance. Merkezi Hekim Randevu Sistemi (MHRS) appointment booking is restricted to YKN-holders with a residence record.
How do I check whether I am already registered?
Log in to e-Devlet at turkiye.gov.tr using YKN and e-Devlet şifresi, then open the Genel Sağlık Sigortası Tescil ve Prim Borcu Sorgulama service. If the page shows tescil under Article 60(d), the status has been auto-registered — only premium payment is required. If no tescil appears within a few weeks of crossing the one-year mark, file the Genel Sağlık Sigortası Giriş Bildirgesi in person at the SGK Müdürlüğü responsible for the ADNKS-registered address. The dependents-query service (sosyal-guvenlik-genel-saglik-sigortalisinin-bakmakla-yukumlu-oldugu-aile-bireyleri-sorgulama) confirms which dependents are linked.
Is there a waiting period before benefits start?
No. Article 67 of Law 5510 ties benefit entitlement under Article 60(d) to the date on which GSS-insured status begins — the threshold date itself — provided the premium for that month is paid within the statutory window. The one-month figure circulating in some popular guides reflects the bildirim (notification) window under Article 61, not a benefit-activation waiting period. Acil sağlık hizmetleri (emergency care) is universal and unconditional irrespective of any premium status.
Can I book an MHRS appointment without SGK enrolment?
Yes. Merkezi Hekim Randevu Sistemi (MHRS) booking is available to any resident with a YKN and a valid residence record — GSS-insured status is not required to book. The consultation itself is billed to SGK for GSS-insured patients and to the patient otherwise. MHRS multilingual phone booking is available via Alo 182 in Turkish, Arabic, English, and Russian; the mhrs.gov.tr portal and the MHRS Mobil app offer the same scheduling for state hospitals, Ağız ve Diş Sağlığı Merkezleri, and aile hekimi offices.
After This Process
- → Register direct debit (otomatik ödeme) for the monthly GSS premium at the bank or via e-Devlet to avoid prim borcu
- → Add each eş, çocuk, or qualifying ana-baba dependent to the SGK record once each holds a YKN and a residence permit; verify in the e-Devlet dependents query
- → Bookmark MHRS for state-hospital and aile hekimi appointment booking; the same login covers all Sağlık Bakanlığı facilities
- → Decide whether to keep the private health-insurance policy in parallel for non-SGK-contracted private hospital access or to rely on GSS only

SGK 2026 announcement: "2026 yılı için Genel Sağlık Sigortası aylık prim tutarı ₺1.981,80'dir (asgari ücretin %6'sı)." The monthly GSS premium under Article 60(d) of Law 5510 is calculated as 6% of the gross asgari ücret — for 2026 (in force from 1 January 2026), 6% × ₺33.030,00 = ₺1.981,80 per month. The rate revises with each asgari ücret step, automatically and without a separate SGK announcement.

Article 60(d) of Law 5510 makes foreign nationals holding a Turkish residence permit compulsory GSS-insureds once their continuous legal residence in Türkiye exceeds one year, provided they are not insured under Article 4 (4/a salaried, 4/b self-employed including the former BAĞ-KUR self-employed regime, or 4/c public officer) and not insured under their country of origin's social-security system. Article 61 sets the bildirim (notification) window of one month from the threshold date. Article 67 ties benefit entitlement to the date GSS-insured status begins, with no separate benefit-activation waiting period. Article 3(10) defines dependents; Article 75 establishes universal emergency care entitlement irrespective of insurance status. Articles 80–82 govern premium-base earnings.

Law 6458 governs the residence-permit precondition for Article 60(d) of Law 5510 — not the GSS-insurance status itself. Article 8 conditions visa issuance on valid health insurance covering the planned stay; Article 23 conditions issuance of a short-term ikamet izni on health-insurance coverage; Article 42 establishes the long-term residence permit after eight uninterrupted years of legal residence. The private health-insurance requirement during year 1 of a short-term ikamet izni derives from Articles 8 and 23 of Law 6458, operationalised through Hazine ve Maliye Bakanlığı Circular 2014/9 (updated by Circular 2021/8).
mevzuat.gov.tr - T1Göç İdaresi Başkanlığı — General Health Insurance for foreigners 2026-05-25
Foreigners holding a Turkish residence permit whose residence has exceeded one year become GSS-insureds under Article 60(d) of Law 5510, provided they are not covered as salaried employees, self-employed, public officers, or under another country's social-security system. The status arises automatically on crossing the one-year mark; bildirim (notification) is filed at the local SGK Müdürlüğü. Foreigners in short-term ikamet izni during year 1 must hold a private health-insurance policy meeting the Hazine ve Maliye Bakanlığı Circular 2014/9 minimum-coverage standard; the policy must accompany every ikamet izni issuance and renewal application during the year-1 period.

The 6% GSS premium for Article 60(d) insureds is calculated mechanically against this gross base — ₺33.030,00 × 0,06 = ₺1.981,80 per month — matching the SGK premium announcement. The base revises annually (typically January, occasionally mid-year).

The policy must carry the standard-form declaration referencing Circular 2014/9 (or 2021/8) verbatim. Coverage minimums include inpatient cover at or above the prescribed limit, geographic scope of Türkiye only, and duration covering the full validity period of the ikamet izni being issued. There is no closed accredited-insurers list — any SEDDK-licensed insurer issuing a compliant policy qualifies.
